Seniors who are waiting in the hospital for a long-term care home or who need more time before returning home require extra help. Our 24-7 intensive Transitional Care Program does just that. By offering services like personal support workers, case management and emergency response in a temporary supportive apartment, seniors can safely make the transition back home or to a new long-term care residence.
Once you are ready, you can go back home or to a long-term care home. Our Case Management team will help you with whatever you need and will make sure you have services to help you at home.
