People who come to us struggle with complex issues, including homelessness, poor physical and mental health, substance use, unemployment and financial insecurity. Our Case Managers assess people's needs, and coordinate relevant services on behalf of the people they serve. This trauma-informed support improves people's short and long-term stability and well-being.

How to apply

Self-referral or provider referrals. Drop-in appointments are available. 

Some of the services provided by our Case Managers include:

  • Risk assessments and de-escalation

  • Non-emergency mental health crisis

  • Referrals to health care providers

  • Harm reduction planning and assistance

  • Housing support

  • Coordinating and supporting Ontario Disability Support Program (ODSP) applications

  • Advocacy within the Social Benefits Tribunal
